In order to remain in compliance with Department for the Aging (DFTA) and NYS Office of the Aging requirements, you MUST complete the Updated Consent Form, linked below.
We were informed by DFTA that any CLLD participant who does not complete and submit the updated form will be deactivated from our system by DFTA and will no longer be allowed to participate in our program. If you have not already completed this form, you can complete it here electronically and return it via email to [email protected] OR stop by the CLLD office to submit the completed form in person to any CLLD staff member.
The deadline for submitting this form either electronically or in person is March 15, 2024.
